SPARC

Short-term Post Acute Rehabilitative Care (SPARC) is one of the many services provided by The Royal Hospital Donnybrook (RHD).  The programme is suitable for older people who are medically stable, fit for discharge from acute care, but who require a further period of short term, multidisciplinary in-patient care during the post-acute phase to optimise recovery and independence.

This multidisciplinary service is characterised by comprehensive assessment, resulting in a structured individual care and treatment plan that involves nursing care, active therapy, treatment and opportunity for recovery. The multidisciplinary team adopts a rehabilitative and enabling approach for each individual patient. Admission to the SPARC programme may facilitate and support timely discharge from acute care.

The length of stay varies for each individual patient but is generally between two and six weeks.

Patients are referred to the SPARC service from any medical or surgical team in St. Vincent’s University Hospital.
